I, William Thomas, appellant, pro se in the above entitled action, hereby declare under penalty of perjury that the following is true and accurate.

1. On October 20, 1996 I posted, by certified U..S. mail, return receipt requested, a Notice of Appeal in the instant case at the Main Post Office in Washington, DC.

2. At that time I received a certified mail receipt for the posted Notice of Appeal, date stamped October 20, 1996. A true and accurate photocopy of that receipt is attached hereto.

3. Subsequently, I received a return receipt indicating that the Notice of Appeal was received by the U.S. District Court in Jacksonville, Florida on October 25, 1996. A true and accurate photocopy of that receipt is also attached hereto.
