IN FORMA PAUPERIS

I, Ellen Thomas, being first duly sworn, hereby declare that I am a petitioner in the above-entitled case, that in support of my motion to proceed without being required to prepay fees, costs or give security therefor, I state that because of poverty, I am unable to pay the costs of this case or to give security therefor; and that I believe I am entitled to redress.

I further swear that the responses which I have made to the questions and instructions below relating to my ability to pay the cost of proceeding in this Court are true.

1. I am presently employed full time in the religious service of peacemaking. I sincerely believe myself to be serving God and humanity. I receive no recompense, or government assistance.

2. During the past twelve years I have not received any income from a business, profession or other form of self- employment, nor in the form of rent payments, interest, dividends or any other sources. The only money that has gone through my hands has been in the form of small unsolicited donations which is almost entirely spent on peace-promoting literature.

3. I have no personal money in any checking or savings account.

4. I own no real estate, stocks, bonds, notes, automobiles, or other valuable property.

5. There are no persons who are dependent upon me for financial support.

I understand that any false statement in this declaration will subject me to penalties for perjury.

b. I was last employed for financial compensation in 1974 as a deckhand on a oil-rig supply boat making, if I recall correctly, $200.00 dollars a week.

2.
